Horizon Therapeutics is studying whether KRYSTEXXA (pegloticase), an IV treatment for gout, can be given once every 4 weeks instead of the current every 2 weeks, while maintaining similar effectiveness. This schedule may reduce the number of visits required. Compensation is available for time and travel.

Who may be a good fit for this study?

This study is designed for adults whose gout has been difficult to control even after trying standard medications. Below are the key things that may (or may not) apply to you.

Adults 18 years or older

This study is open to any adult man or woman aged 18 and above.

Gout that hasn't responded to standard pills

Your gout continues despite trying medications like allopurinol or febuxostat at the highest dose your doctor recommended or, you can't take those medications for medical reasons.

Recent gout attacks or visible lumps

You've had 2 or more gout flares in the past year, or you have visible chalky lumps (called tophi) caused by uric acid crystals building up under the skin.

Weight under 352 lbs (160 kg)

The study medication has been tested and dosing is calibrated safely for people within this weight range.

Recent serious heart events

People who were hospitalized for heart failure, or treated for a heart attack or serious unstable chest pain in the past 3 months, cannot join for safety reasons.

Long-term strong immune-suppressing medication

People currently taking immune-suppressing drugs like azathioprine or mycophenolate, or high-dose steroids (prednisone over 10 mg daily for 3 months or more), are not eligible at this time.

You can be compensated for your time

All gout-related care during the study is completely covered — no out-of-pocket costs, and no health insurance required. You'll also receive compensation for your time and any travel involved in attending visits.

Weeks 1-4 · 2-3 clinic visits

Medical screening & preparation

You'll visit the clinic in Chicago for a thorough health review, blood tests, a physical exam, and an ECG. If all looks good, you'll start a weekly oral medication for 4 weeks to prepare your immune system for the main treatment phase.

Months 2-12 · up to 18 IV infusions

Treatment period

You'll receive IV infusions of the study medication at the clinic, roughly every 2 or 4 weeks depending on the group you're assigned to. Each session lasts about 2 hours. You continue the weekly oral medication throughout. During the second half, everyone receives the every-4-week schedule.

Months 13-16 · 2 follow-up contacts

Safety follow-up

After your last infusion, the team will check in by phone or email 30 days later. Then, about 3 months after your final dose, you'll come in for one last safety visit at the clinic. After that, you're done!
